Medical Declaration
- I declare I am physically fit to participate
- I understand I should seek medical advice if unsure
- I will inform organisers of relevant medical conditions
- In the event of injury or illness, I consent to receiving first aid and, if necessary, medical treatment, including ambulance transport
Warning under the Competition and Consumer Act 2010 (Cth)
This waiver is intended to exclude liability for breach of statutory guarantees under the Australian Consumer Law in relation to recreational services.
Under section 139A of the Competition and Consumer Act 2010 (Cth), a supplier of recreational services may exclude liability for death or personal injury resulting from a failure to comply with statutory guarantees.
By signing this document, I acknowledge that:
- My rights (or the rights of my dependents) to sue the Providers for breach of the statutory guarantees may be excluded, restricted, or modified as permitted by law; and
- This exclusion does not apply to conduct involving recklessness

Participation in pickleball is a recreational activity involving obvious risks including:- Slips, trips, falls, or collisions
- Physical exertion leading to injury or illness
- Aggravaton of pre-existing medical conditions
- Actoins of other participants
- Injuries from contact with equipment or court surfaces
Legal Acceptance
- I acknowledge the Tasmanian risk warning
- I understand I participate at my own risk
- I accept all obvious risks associated with pickleball
- I release Sorell Pickleball Club and organisers from liability to the extent permitted by law
- I understand this includes negligence to the extent permitted by law
- I understand my rights under Australian Consumer Law are not excluded
